The Access to Health Care Community Grant program was designed to support organizations that provide direct health care services, including mental health, dental, and hospice or palliative care services, to those who would not otherwise have access to such services due to economic, social, or geographic reasons.

FAQs
Grants are awarded to charitable organizations providing basic and preventive health care to individuals who would not otherwise have access to such services.
If my organization received an Access to Health Care grant last year, am I eligible to apply this year?
You may reapply this year if the following conditions have been met:
- The funds have been fully expended
- All follow-up grant reports have been submitted
- The program successfully met its goals
My organization has a current grant through one of the other Community Grant opportunities (Opportunities for Children or Services for Older Adults.) Am I eligible to apply for the Opportunities for Children grant?
Yes, but only if it is for a different program. We will not consider funding the same program with two different community grants.
Can an organization submit a Letter of Interest or grant application for more than one project during a single grant cycle?
Yes, applicants will submit letters of intent online. (Please refer to the guidelines above). If your proposed program and organization appear to meet the guidelines, you will receive an email with a link and access code to the online application.
Are applications submitted online?
Yes!

Will the organizations that apply receive notification if they are not approved?
Yes, all organizations that submit an application will be notified of funding decisions by the Notification of Funding Decision by the end of February 2024.
What is the logic model which is referenced in the grant application and follow-up reporting?

Center for Children & Families – $15,000 to provide counseling services to families experiencing child abuse.
Oklahoma Children’s Health Foundation – $6,300 to provide an intensive group treatment program for children with selective mutism.
Community Health Centers, Inc. – $35,000 to provide Screening Brief Intervention and Referral to Treatment (SBIRT) for uninsured patients at Community Health Highland Park Family Medical Center.
Crossings Community Clinic – $20,000 to support uninsured patients with the management of uncontrolled diabetes to achieve better overall health.
Health Alliance for the Uninsured, Inc. – $40,000 to provide access to free prescription medications and flu shots for low-income, uninsured adults.
Limbs for Life Foundation – $15,000 to provide up to six people with prosthetic limbs.
Neighborhood Services Organization, Inc. – $35,000 to provide 8,000 procedures to address chronic and preventative care needs.
NewView Oklahoma, Inc. – $7,500 for essential low-vision services.
Oklahoma Assistive Technology Foundation – $20,000 to provide no-cost durable medical equipment to Oklahomans in need.
Oklahoma Dental Foundation – $10,000 to partner with D-DENT and Dale Graham Veterans Foundation to provide dental care and treatment to their clients.
Parent Promise – $15,000 to provide postpartum care products to women who are first-time mothers in high-risk situations.
Planned Parenthood Great Plains – $35,000 to provide affordable care to 200 patients if Title X benefits do not apply.
Skyline Urban Ministry – $8,700 to provide 250-300 patrons seeking vision correction services.
Teen emPower, Inc. – $40,000 for implementing teen pregnancy prevention education in 8th-grade classrooms.
Youth and Family Services, Inc. – $20,000 to provide a full-body approach to recovery focusing on exercise, nutrition, counseling, and therapeutic activities that promote self-care.

NewView Oklahoma – $10,000 – To expand their low-vision optometry and rehab services to patients in non-metro areas of Oklahoma, Cleveland and Canadian Counties.
Oklahoma Dental Foundation – $20,000 – To provide on-site dental services to Oklahoma veterans utilizing their mobile dental units.
Oklahoma Project Woman – $15,000 – To provide breast health care for uninsured Oklahomans.
Planned Parenthood Great Plains – $30,000 – To provide under and uninsured patients with no-cost reproductive and primary care.
Remerge of Oklahoma County – $18,000 – To partner with Mobile Smiles to provide onsite dental services for ReMerge participants.
Sunbeam Family Services – $30,000 – To establish a relief fund to help mental health patients afford and continue care.
The CARE Center – $15,000 – To provide trauma-informed mental health services for children in Oklahoma City.
The Oklahoma Caring Foundation – $10,000 – To support the Caring Van units that provide free immunizations to Oklahoma City children.
Variety Care – $15,000 – To provide oral health care to uninsured human trafficking survivors.
YMCA of Greater Oklahoma City – $8,000 – To start a Pedaling for Parkinson’s cycling program to help treat and slow the progression of symptoms for people living with Parkinson’s disease.
